A Family, School, or General Social Worker provides specialized assistance to vulnerable families and children. Intervenes in crisis situations; evaluates individual and family needs, helps clients get access to health care, mental health care or social services such as housing assistance. Advocates for clients and provides follow up visits. May work with teachers, parents and school administrators to help students improve academically and deal with social issues such as bullying.

Job Description
Responsibilities and Accountabilities:
- As a social worker, have knowledge and understanding of Social Work practices and principles
- Responsible for effective case psychosocial assessment and suicide risk assessment, discharge planning, and leading psychoeducational and/or therapeutic groups
- Responsible for communicating with patients and families to keep them involved in treatment plan decisions
- Coordinates care with outpatient providers upon admission and discharge
- Contributes clinical perspective to the interdisciplinary team based on unique training in the area of family systems and/or social work theory.
- As a therapist, facilitates group, individual, and family therapy sessions
- Will be available to provide coverage for other social workers/therapists when absent or as the need arises
- Communicate clearly and effectively with clinical team members and Directors
- May be assigned supervision responsibilities for those pursuing clinical licensure
- Is oriented and educated in gender, cultural differences, and age appropriate care for the assigned department
- Must be flexible to work at different sites within the PrairieCare metro locations.
Great Fit Education:
Master's degree and licensed or license eligible within 6 months of hire as a mental health professional in the State of Minnesota.Qualifying licensures include:
Licensed Independent Clinical Social Worker (LICSW), Licensed Marriage and Family Therapist (LMFT) or a Licensed Professional Clinical Counselor (LPCC) or Licensed Psychologist (LP).Previous Experience:
- Two years previous mental health experience strongly preferred. Experience in intensive psychiatric healthcare settings preferred
- Experience using electronic health records preferred Compensation for this role ranges from $X-X per hour or annually depending on education and experience.
