Job Description

SUMMARY The Licensed Social Worker•Community Care & Resource Coordinator exists to help Aspen Grove churches faithfully and effectively respond to the real needs of individuals and families in their communities while ultimately pointing people toward holistic flourishing in Christ.? This role addresses housing instability, food insecurity, financial hardship, mental health concerns, relational challenges, and access to essential resources, while helping individuals experience restoration, belonging, and transformation and full flourishing through Jesus and His Church.? The role prioritizes connecting people first to church-based care and resources before engaging external systems when needed.?
QUALIFICATIONS
BSW or MSW degree Licensed in Minnesota (LSW, LGSW, LISW, or LICSW) or pursuing licensure Experience in case management and financial/housing assistance Knowledge of Minnesota social service systems
RESPONSIBILITIES
Conduct assessments and provide case management Partner with school counselors and others to identify at-risk families Connect individuals first to church-based care and support systems Navigate external resources when needed Build partnerships with community agencies Develop care systems across churches Other Responsibilities as directed by your supervisor.
