Job Description

JOB DETAILS POSITION SUMMARY
  • LSW Social Worker
POSITION DUTIES
  • Provides services to customers and their families/significant others, to help them adjust to illness, injury or hospitalization.
Assist the health team in understanding the significance of social, economic, emotional and family factors in relation to the customer's illness, treatment, recovery or dying processes. Responsible for facilitating timely, efficient, and appropriate discharge of customers and coordination with next level of care, including transmission of appropriate plan of care. Identify needed community services and coordinate appropriate services for customers in need.
MINIMUM REQUIRED QUALIFICATIONS
  • LSW in the state of Minnesota 2+ Years Hospital Social Working Experience Discharge Planning Experience All Patient Populations Experience
PREFERRED QUALIFICATIONS
  • Epic experience
LENGTH OF ASSIGNMENT
  • 8 weeks
SHIFT / HOURS PER WEEK
  • 8:00 AM
  • 5:00 PM with every 6th weekend. SYSTEMS
