Urgent need for a Social Worker
LMSW for one of our client which is a Hospital.

Job Location:

Maple Grove, MN 55369

Shift:

Days(8:00 AM

4:30 PM)

Duration of Contract:

13 weeks +

Extendable Weekend Requirements:

Every 3rd

Weekend Guaranteed Hours:

40hrs/week Minimum years of experience: 2 years Pay rate: $53/hr Assisting clients in receiving services by telephone and in person. Getting clients involved in beneficial activities. Assessing clients and gathering relevant information. Offering information and supporting clients and their families. Contacting and making referrals to other agencies and services. Maintaining accurate records and preparing reports.

Position Requirements:

2+ Years Hospital Social Working Experience

Required Acute Care (Hospital) Discharge Experience
Required Adult and Geriatric Experience
Required BSW, CSW, or MSW
Required BLS
Required Must have Social Worker License in MN Must have MN State License.
