Job Summary Parkriver Healthcare & Rehabilitation currently has an opening for a Long Term Care Social Worker.

A DAY IN THE LIFE OF A SOCIAL SERVICES DESIGNEE

As a Social Services Designee, you play a vital role in supporting residents' emotional, social, and behavioral well-being. Each day, you help ensure residents feel heard, supported, and connected.

Your daily responsibilities include:

Conducting psychosocial assessments to evaluate resident emotional and social needs Providing emotional support, active listening, and crisis intervention when needed Advocating for resident rights, preferences, and person-centered care Developing and updating individualized service plans focused on mental health and social needs Coordinating with families, guardians, mental health providers, and outside agencies Monitoring changes in resident mood, behavior, and functioning Supporting behavioral interventions and care planning initiatives Assisting with admissions, transitions, and discharge planning to ensure continuity of care Maintaining accurate documentation in compliance with facility policies and state regulations Must be Licensed Social Worker in the state of MN. Your ability to connect with residents, collaborate with care teams, and advocate for individualized support is what makes this role essential to our mission.
