None Position Overview We are seeking an experienced Licensed Clinical Social Worker (LCSW) to join a multidisciplinary hospital care team in Robbinsdale, Minnesota. This position is ideal for a compassionate and skilled social worker with strong acute care experience who is passionate about patient advocacy, discharge planning, and care coordination across diverse patient populations. The successful candidate will work collaboratively with physicians, nurses, case managers, and community agencies to ensure safe, timely, and effective transitions of care. Key Responsibilities Conduct comprehensive psychosocial assessments for hospitalized patients and their families. Develop and coordinate individualized discharge plans to ensure safe transitions to home, rehabilitation, skilled nursing facilities, or other levels of care. Collaborate with physicians, nurses, case managers, therapists, and interdisciplinary healthcare teams to address patient needs and discharge barriers. Provide crisis intervention, counseling, emotional support, and patient advocacy. Coordinate referrals to community resources, behavioral health services, home health agencies, hospice, rehabilitation facilities, and other post-acute care providers. Assist patients and families with advance care planning, financial resources, insurance issues, and social service needs. Maintain accurate, timely documentation in the Epic electronic medical record system. Participate in interdisciplinary rounds and contribute to patient-centered care planning. Ensure compliance with hospital policies, HIPAA regulations, and state and federal healthcare standards. Provide weekend coverage as scheduled. Requirements Active Minnesota Licensed Social Worker (LSW) License

Excellent communication, assessment, crisis intervention, and care coordination skills. Ability to manage multiple priorities in a fast-paced hospital environment. Preferred Qualifications Previous experience in acute care case management or utilization management. Knowledge of Medicare, Medicaid, managed care, and community resource coordination.
