Responsibilities:
Professionalism & Work Habits:
1. Maintain professional conduct consistent with agency values, including ethical decision-making, confidentiality, appropriate boundaries, and respectful interactions. 2. Demonstrate reliability, punctuality, accountability, and the ability to manage time, travel, and caseload demands effectively. 3. Follow agency policies, procedures, and supervisory directives. 4. Manage stress effectively and maintain composure in high-pressure or crisis situations.
Clinical Responsibilities:
1. Implement the Wraparound process for each assigned child and family, including obtaining Releases of Information, facilitating Family Support Team meetings, and developing individualized, strengths-based care plans. 2. Conduct comprehensive assessments addressing child safety, risk of abuse or neglect, family functioning, strengths, needs, domestic violence concerns, and other factors impacting treatment planning. 3. Apply trauma-informed, evidence-based, and culturally responsive practices in assessment, care planning, and service coordination. 4. Maintain required weekly face-to-face contact with children and caregivers, requiring extensive travel and the ability to drive for extended periods. Must be able to drive a minimum of 8 hours/day and must be able to operate a motor vehicle throughout employment. 5. Coordinate, facilitate, and/or supervise visitation in accordance with FST recommendations and court orders. 6. Provide crisis intervention and develop safety plans that are actively utilized by youth, families, and providers. 7. Participate in on-call rotation to meet the needs of clients and families outside standard work hours when required. 8. Engage families who may be reluctant or resistant to services using effective engagement strategies. 9. Facilitate permanency and family connections through family search, engagement, and involvement of natural supports. 10. Monitor service effectiveness, evaluate progress toward individualized and permanency goals, and adjust plans as needed. 11. Support transitions, step-down planning, and disenrollment to ensure continuity of care and follow-up supports. 12. Prepare court documentation, make recommendations regarding services and permanency, and testify in court as required. 13. Seek supervisory guidance appropriately when clinical risk or complexity increases. 14. Comply with all Federal and State laws and organizational policies and procedures.
Communication & Collaboration:
1. Communicate clearly, professionally, and proactively with supervisors regarding significant case events, risks, or barriers outside of scheduled meetings. 2. Present case information clearly and professionally in meetings, written reports, and court proceedings. 3. Collaborate effectively with families, providers, internal team members, and community partners. 4. Use effective crisis-communication and conflict-resolution skills.
Documentation & Administrative Responsibilities:
1. Complete all required documentation accurately, professionally, and within established timeframes, including case notes, FST documentation, court reports, medical documentation, authorizations, placement updates, and required state system entries (e.g., CSWIS). 2. Coordinate child and family support providers and ensure provider skills align with individualized care plan needs. 3. Maintain client and staff files in compliance with agency standards. 4. Meet performance benchmarks related to documentation timeliness, service coordination, and outcome measures. 5. Demonstrate responsible use of resources and compliance with budget expectations. 6. Attend supervision, staff meetings, trainings, Family Support Team meetings, conferences, and court hearings. 7. Maintain reliable communication, including use of a company-issued cell phone. 8. Perform other duties as assigned.
Additional Responsibilities:
As part of the MACF team, Care Managers may be asked to assist with Care Management and Resource Development activities to support youth and families.
Qualifications:
1. Master's degree in psychology, social work, counseling, criminal justice, or related field; OR 2. Bachelor's degree in a human behavioral science plus two years' experience in protective services, family preservation, or targeted case management. 3. Strong written and verbal communication skills and ability to complete paperwork accurately and on time. 4. Proficiency in word processing and data entry; access to a personal computer. 5. Knowledge of Missouri child welfare systems and community resources. 6. Valid driver's license, acceptable driving record, reliable transportation, and vehicle insurance. 7. Ability to meet physical travel requirements (up to 8 hours of driving per day). 8. Clear background check and freedom from communicable diseases.
