Now Hiring:
Hospice Social Worker - Licking, MO and West Plains, MO Three Rivers Hospice is seeking a compassionate, dedicated Social Worker to join our team serving the Licking, Missouri area. If you are passionate about making a meaningful difference in the lives of patients and families facing a terminal illness, we'd love to hear from you. As a Hospice Social Worker, you will provide emotional support, counseling, advocacy, and access to community resources for patients and their loved ones. This position offers the opportunity to work in a variety of settings, including hospitals, senior care facilities, and patients' homes, while collaborating with an interdisciplinary hospice team to provide exceptional end-of-life care.
Responsibilities:
Conduct comprehensive psychosocial assessments to identify patient and family needs. Develop and implement individualized care plans and interventions. Provide crisis intervention and emotional support during difficult situations. Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated patient care. Connect patients and families with community resources and support services. Advocate for patients' rights and help families navigate healthcare and social service systems. Maintain accurate documentation in accordance with hospice regulations and agency policies.
Qualifications:
Bachelor's degree in Social Work required; Master of Social Work (MSW) preferred. Knowledge of medical terminology and medical documentation practices. Excellent verbal and written communication skills. Ability to work independently and as part of a multidisciplinary team. Strong organizational skills and a commitment to maintaining patient confidentiality. A compassionate, patient-centered approach and dedication to ethical social work practice. $65,000.00 - $68,000.00 per year
