Job Description

Position Overview This position is responsible for providing in-home instruction as well as a group socialization experience to parents and children. Home Visitor is responsible for maintaining on-going contact with families and work with all other components (health, nutrition, family services, mental health, special services, parent education and resource/referral) to integrate services into the family setting. Key Responsibilities Administration, Planning and Evaluation Conduct weekly 90-minute home visits with each family. Provide health, educational, nutritional and child growth and development information to parents via weekly home visits. Coordinate with parents to ensure that prenatal, well-child exams, immunizations, health screenings, and nutrition assessments are complete and follow up occurs. Complete family partnership agreements, developmental screenings, on-going assessment and all other required assessments with families in a timely manner. Assist and support families in locating resources and scheduling appointments with community resources that best meets their needs. Provide support in parent and child activities that encourages developmentally appropriate activities for the changing needs of infants and toddlers. Provide bi-monthly socializations for infants/toddlers and their primary caregivers. Ensure group activities to reflect the unique needs and strengths of all children in the classroom. Assist parents in strengthening their involvement in their child's learning by creating a home learning environment, participating in group socialization activities and attending parent committee meetings. All other duties as assigned. Quality and Community Relationships • Maintain good communication with program staff, families and community/ agency businesses • Establish and maintains high quality services to program staff and families Supervisory and Staff Relationships There are no supervision requirements of this position. Cooperate with other staff and program leadership to ensure high quality service provision Participate in and attends staff meetings, and other meetings; as needed Qualifications Experience with working with low income individuals/families. Ability to prioritize and self-motivate to complete tasks. Clear and effective written and spoken communication skills. Knowledge of community resources Proficient with Microsoft Office Suite or related software.
Physical Requirements:
Prolonged periods of sitting at a desk and working on a computer. Must be able to lift 20 lbs. at times. Education & Experience Home Visitor CDA or Family Development Credential. A minimum of two years working with young children and low-income families.
