Job Description

Job Overview We are seeking a dedicated and compassionate Licensed Social Worker (LSW) to join our team at a reputable skilled nursing facility. In this vital role, you will provide comprehensive social work services to residents, supporting their emotional well-being and ensuring they receive personalized care tailored to their unique needs. Your expertise will help foster a nurturing environment, promote recovery, and enhance quality of life for our residents. This position offers an exciting opportunity to make a meaningful difference in the lives of seniors and individuals with complex health conditions. Responsibilities Conduct thorough patient assessments, including psychosocial evaluations and diagnostic evaluations, to develop individualized care plans. Provide clinical counseling, psychotherapy, and behavioral therapy to residents experiencing mental health challenges such as dementia, Alzheimer's disease, PTSD, or developmental disabilities. Facilitate group therapy sessions and individual counseling to support emotional resilience and mental health stability. Coordinate with interdisciplinary teams to ensure seamless case management, discharge planning, and post-acute care transitions. Maintain accurate medical documentation and records in compliance with HIPAA regulations. Assist with crisis intervention and crisis management strategies to address urgent behavioral health concerns effectively. Collaborate with families during intake processes and provide family counseling or child & family counseling when appropriate. Engage in data collection for clinical research projects related to behavioral health or senior care initiatives. Work within EMR (Electronic Medical Records) systems to document patient care activities thoroughly and efficiently. Qualifications Valid state licensure as a Licensed Social Worker (LSW) with relevant certification for healthcare settings. Proven experience working in skilled nursing facilities. Excellent communication skills for patient assessment, family engagement, and interdisciplinary collaboration.
Pay:
$40,000.00 - $48,000.00 per year
