Job Description
We are seeking a compassionate and dedicated Hospice Social Worker to join our team. The ideal candidate will have a strong background in geriatrics and hospice care , with experience in an outpatient setting . This role involves delivering a wide range of social work services to hospice patients and their families, including: Conducting initial emotional, spiritual, and psychosocial assessments Providing ongoing counseling and support Offering bereavement services Participating in community education, outreach, and referrals As a key member of the Interdisciplinary Group (IDG) , the Hospice Social Worker plays an integral role in assessing patient needs, developing individualized care plans, and ensuring compliance with hospice care standards.
