Substitute Verification Form 5. Official Transcript/ High School Diploma/GED Once all pre-employment documents are completed, you will receive another electronic packet of forms consisting of payroll documents and onboarding information. 1. Direct Deposit 2. NC-4 3. W-4 4. Worker's Compensation Information 5. I-9 Form 6. Email Information 7. Copy of Driver's License & Social Security Card 8. Onboarding Information Step 3.
