Social Worker Case Manager Jordan Management Group - 3.3 Greenville, NC Job Details Full-time 12 hours ago Qualifications Teamwork Driver's License Bachelor's degree Social Work Patient observation Full Job Description Jordan Management Group is in need of a full-time Social Worker Case Manager. Jordan Management Group, LLC is a Human Service Organization that is committed to serving children and adults with long-term care needs. Since our inception in 2009, our mission is to provide person centered quality services to consumers and their families within a community setting for an improved quality of life. The Social Worker Case Manger will work in collaboration with a Registered Nurse to evaluate and manage children participating in the Community Alternatives Program for Children (CAP/C). CAP/C is a Medicaid waiver program designed for medically fragile children at risk for institutionalization. The program provides a variety of services to enable the children to remain in their homes.
The Social Worker Case Manager:
  • Conducts a comprehensive, multi-disciplinary, person-centered assessment to determine continued eligibility for the program and drive plan of care development. These assessments are done during a home visit, and are conducted via interview of caregivers, observation of the child, and review of medical records;
  • Consults and collaborates with a Registered Nurse regarding the patient's health care and home care needs;
  • Develops a family-centered plan of care to address the needs identified in the assessment;
  • Links the child/family to services, equipment, and supplies needed to care for the child at home;
  • Monitors the status of the child/family and the provision of services and supplies via monthly contacts with the family and certain service providers, and quarterly visits and multi-disciplinary team meetings;
  • Performs change of status assessments, with the RN when warranted, when there is a significant change in the patient's health condition and/or home care needs;
  • In collaboration with the RN, follows patients post-hospitalization to ensure needs are met and prevent re-hospitalization;
  • In collaboration with the RN when warranted, follows-up critical incidents and addresses prevention of future incidents;
  • During team meetings, provides input regarding other patients served by JMG;
  • Electronically documents all activities in accordance with JMG and Medicaid regulations;
  • Learns and keeps up-to-date regarding CAP/C policy, Medicaid policy, and other resources for medically fragile children and their families;
  • Participates in Quality Management Activities as needed;
  • Maintains a productivity level of billable hours per week; and
  • Maintains compliance with required activities as described in the CAP/C policy.
This is an hourly position. Hours ae generally 8:30 AM to 5:00 PM Monday-Friday, but will vary according to client needs and will include some evening hours. Visits cover a multi-county area and can be as much as two hours away. Mileage is reimbursed. There are no insurance or other benefits offered with this position. Applicant MUST meet one of the following two sets of qualifications. This is non-negotiable. 1. A Bachelor's degree in Social Work from an accredited school and 1 year of community experience (preferably case management) in the health or medical field directly related to home-care, long-term care, or personal care. 2. A Bachelor's Degree from an accredited school and 2 years of community experience (preferably case management) in the health or medical field directly related to home-care, long-term care, or personal care. Pediatric experience preferred. Applicants should be passionate about their work, show initiative, be able to work productively from home, and be able to work collaboratively within a team. Applicants should be able to pass a criminal background check and drug screen. Applicants must have an unrestricted driver's license and a reliable and insured vehicle that can be used to travel to home visits within a multi-county area.
