Work is performed under the moderate supervision of the Human Services Coordinator. Care Management for At-Risk Children (CMARC) program consists of care management for children, birth to five years of age, who have long-term medical conditions, are in long-term stressful situations (been exposed to adverse childhood experience or adversely affected by social determinants of health), children in foster care, and currently/past Neonatal Intensive Care Unit (NICU) admission. CMARC care managers apply systems and information to improve care and assists members and their families in becoming engaged in a collaborative process designed to manage medical, social and behavioral health conditions more effectively. Care managers provide a variety of services in the form of population management and direct care management, as integral members of the child health care team. Once identified and actively receiving CMARC services, members must have clear documentation of a Comprehensive Needs Assessment, interactions, care plan and goal(s) and other Care Management for At-Risk Children activities. Specifically, CMARC activities done with or on behalf of members are recorded as interactions, including outreach to members to engage them in care management. Engagement into the program is voluntary for the child and family. Position Responsibilities Domain 1: Data Analytics and Assessment Collect, analyze, and manage the information gathered during the assessment process to determine whether the member meets the target population description and to determine the need for and the level of service to be provided. Identify and describe factors that affect the health of the target population. Access, collect and evaluate quantitative and qualitative data using various methods and tools to monitor outcomes and positively impact performance measures. Domain 3: Communication Skills Facilitate communication among individuals, groups, and organizations. Utilize motivational interviewing skills to guide members in developing patient-centered goals with individualized interventions that move the member toward optimal health and wellbeing. Educate member on diagnosed conditions, psychosocial needs, and community resources to positively influence behavior and improve health while encouraging self-management through materials that meet literacy standards. Domain 4: Health Equity Skills Apply principles of ethics, diversity, equity, inclusion, and justice. Engage in continuous self-reflection and identify how biases influence policies, programs, and services. Recognize the diversity of individuals and populations. Engage in professional development to stay current in evidence-based practices. Domain 6: Public Health Sciences Identify public health programs and community organizations with authority to address specific health needs of the target population. Apply public health sciences in delivering the 10 Essential Public Health Services. Advocate for patient health and wellbeing by recognizing the diversity of individuals and involving the individual in the decision-making process through a patient-centered & collaborative partnership. Coordinate care across care team members and maintain regular communication to improve member's care and ensure public health services are provided in a coordinated effort that does not duplicate services. Contribute to the development of organizational strategic plan for continuous improvement. Domain 8: Leadership and Systems Thinking Skills Achieve program goals in accordance with program guidelines, including condition-specific pathways, utilizing those interventions that are most effective in engaging members, meeting their needs, and achieving care plan goals. Apply critical thinking in the decision-making of services to evaluate, identify and address medical and non- medical drivers of health. Perform assigned functions as a Disaster Ready Team (DRT) member, if appointed or volunteers. Perform other duties as assigned. Qualifications Considerable knowledge of social work principles, techniques, and practices and their application to individual casework group work and community problems. Knowledge of the psychosocial, socioeconomic, and behavioral problems and their treatment. Knowledge of governmental and private organizations and community resources. Knowledge of laws, regulations and policies which govern the program. General knowledge of medical terminology, disease processes and their treatment are required in certain programs or settings. Skill in establishing rapport with a client and applying techniques of assessing psychosocial. Behavioral, and psychological aspects of client's problem. Ability to establish and maintain effective working relationships with administrative superior members of caseload and their families, and with civics medical, social, and religious organizations. Ability to train or instruct lower-level social workers, students, or interns. Ability to express ideas clearly and concisely and to plan and execute work. Ability to operate a personal computer, standard office productivity software and proficiency with the technologies required to perform care management functions. A high level of professionalism and possess appropriate skills needed to work effectively with the target population at high risk for adverse health outcomes. Ability to effectively communicate with families and providers. Critical thinking skills, clinical judgment, and problem-solving abilities. Education, Experience, and Special Requirements Employ care managers meeting
CMHRP/CMARC
competencies defined as having at least one of the following qualifications: i. Social workers with a Bachelor of Social Work (BSW, BA in SW, or BS in SW) or Master of Social Work (MSW, MA in SW, or MS in SW) from a Council on Social Work Education (CSWE) accredited social work degree program. ii. Registered nurses iii. Bachelor's degree in a human service field with 5 or more years of care management/case management experience working with the specific population of low-income, pregnant individuals and/or children ages 0 to 5 years iv. Bachelor's degree in a human service field with 3 or years of care management/case management experience working with the specific population (low income, pregnant individuals and/or children ages 0 to 5 years and has certification as a Case Manager (CCM preferred)
Note:
Case managers providing services to the
CMHRP/CMARC
population hired prior to Sept. 1, 2011 without a Bachelor's or Master's degree in Social Work may retain their existing positions only. This grandfathered status does not transfer to any other position.
Special Requirements:
Must possess a valid driver's license. Salary will be determined based on qualifications, internal equity, budget, and market considerations. Regular Onslow County employees have the ability to participate in a wide array of employee benefits including: Health Insurance Health Insurance is provided by BlueCross BlueShield of North Carolina and covers a wide variety of healthcare services. Dental Insurance Dental insurance is provided by Delta Dental to full-time employees and covers a percentage of basic to major dental procedures, including orthodontia for children and adults. Vision Insurance Vision insurance is provided by MetLife to full-time employees and covers a variety of materials (contacts and glasses frames/lenses) and an annual eye exam. North Carolina Local Governmental Employees' Retirement System Eligible employees are automatically enrolled in the retirement system and must contribute 6% of their gross salary each pay period. Once eligibility requirements are met, employees will be able to receive monthly payments for life based on their salary, age, and years of service credit. NC401k The County contributes 1% of employees' gross salary into the 401(k) account regardless of the employee's deferral amount. In addition, the County matches employee deferrals up to an additional 4%. Vacation Leave Regular full-time employees earn 13.5 days of vacation leave per year. With each 5-year increment of employment, vacation accrual increases. Sick Leave Regular full-time employees earn 12 days of sick leave per year. Paid Holidays County employees receive paid holidays annually. The Board of County Commissioners approves and may adjust the schedule as deemed necessary. ComPsych Mental Health Services The County provides Mental Health and Life Services through ComPsych to help employees and their families with personal, family and job-related problems such as counseling, crisis support, work-life resources, legal and financial consultations, and organizational support tools.
