The Housing Social Worker provides outreach and engagement, intensive case management and group support to guests experiencing homelessness at the 234-bed South Wilmington Street Center (SWSC). This position is part of a multidisciplinary team delivering substance abuse services, mental health support, job coaching and job skills training, veteran services and case management to support guests in transitioning to stable housing. This role conducts assessments to obtain relevant social history and identify guest needs and connects individuals to appropriate social, medical, legal, financial, employment, behavioral health and housing resources.
Key Responsibilities:
Provide ongoing case management services and support guests in developing individualized service plans focused on achieving housing stability and self-sufficiency Maintain accurate, complete and timely documentation of all guest interactions and services provided Monitor guest progress and compliance with Center policies and program expectations Facilitate or support group sessions and programming for guests Collaborate with internal staff and community partners to coordinate services and support successful housing outcomes
Additional Expectations:
Working knowledge of substance abuse, mental health and trauma-informed care practices Ability to assess needs and connect guests to appropriate community resources Familiarity with local service providers and referral processes
Note:
Due to the nature of operations at the South Wilmington Street Center, staff may be required to work alternate shifts as needed. Schedule changes may occur with reasonable notice when possible.

A bilingual Spanish skills assessment will be administered during the interview. Not only do more units of affordable housing need to be built, housing services must be provided to support families and individuals to access or maintain safe, decent, affordable housing. Affordable housing is also critical to preserving Wake County's economic competitiveness by offering housing for workers at all income levels, supporting housing stability and economic opportunity for its residents and furthering Wake County's commitment to healthy and inclusive growth. The Department's programs include: Community Revitalization, Development Financing, Homelessness and Prevention Services, Emergency Shelter, Support Services and Permanent Supportive Housing. The Basics (Required Education and Experience) Bachelor's degree in Social Work or related field One year of job-related experience Valid Driver's License and "safe" driving record Equivalent education and experience are accepted Please include ALL prior work experience on your application and resume.
