JOB DETAILS POSITION SUMMARY
•LMSW Social Worker
POSITION DUTIES
•Assists with patient intake and scheduling support, insurance and financial assistance coordination, transportation arrangements, community resource referrals, discharge and transition planning, patient advocacy, documentation, and communication between patients, families, and the dialysis care team. Float to sister facilities.
MINIMUM REQUIRED QUALIFICATIONS
•ND LMSW or
LCSW BLS
1+ year experience in specialty
LENGTH OF ASSIGNMENT
•13 weeks
SHIFT / HOURS PER WEEK
•8a-5p, depending on travel or tele visit options for sister locations SYSTEMS•Epic
