Director of Social Services PRIMARY JOB FUNCTION
To provide assessment of psycho-social needs of residents and family members (as needed), and to provide social services to address the needs identified. Services shall address problems of aging, illness, disability, family relationships, spiritual life, and other personal and environmental difficulties. Coordinate a continuity of care from pre-entrance through admission, adjustment, and discharge. ESSENTIAL JOB FUNCTIONS
Will be aware of community health and social resources and be aware of agency functions, eligibility requirements for service, and aid and will offer assistance as needed in these areas. Will be responsible to establish constructive working relationships with members of other professional disciplines in a nursing facility environment. Must demonstrate organizational skills in implementing social service policy and procedures for residents. Responsible to follow established policy and procedures in the social service department and facility wide. Presents one self in a neat and alert manner and quick to take initiative. Must demonstrate ability to responds with good judgment and in a calm manner when under stress. Communicates effectively with the interdisciplinary care team members, residents, families and management in carrying out the duties of this position. Is able to follow instructions, carry out procedures and assume responsibility with accuracy. Collects information and develops a social service assessment on each resident. Participates in resident care management to include the following: Resident care conferences Assists in development on a multi-disciplinary plan of care as needed. Reviews and revise as necessary, the plan for social service intervention. Attends and participates in Unit meetings and Social Service Departmental meetings regularly. Assists in developing departmental standards and policies and evaluate compliance. Performs the following duties in the absence of the social service designee and/or other social worker: Identify social and emotional needs of residents and advising other staff members of the social aspect of a resident's care. Act as a resident advocate, making sure resident's rights are being met. Serve as a liaison for the resident with family, facility and community. Makes referrals to appropriate social agencies as needed. Counsels families in various problem situations concerning nursing facility living. Completes discharge plan/summary Serves as liaison between resident and staff, family and staff, family and resident, resident and resident, etc. as problems arise. Coordinates resident behavioral issues meetings on assigned units. Assists with completion of admission process as assigned. Assists residents and/or families in completing necessary medical forms for services, assistance, etc. when appropriate. Must conduct self with integrity, respect for others and converses with staff, residents and families in a caring, empathetic manner. Must have visual acuity and the ability to read, write, verbalize and understand English well enough to give and receive oral and/or written instructions and successfully communicate with residents and co-workers and complete thorough documentation of social service interventions and assessments in the resident's permanent medical record. Hearing acuity must be sufficient to communicate effectively with residents, staff, health care professionals and family members; also must hear and speak sufficiently to use the phone on a frequent basis. Demonstrates integrity, calm manner & respect for others in interactions with residents, family members, Missouri Slope employees, outside health professionals and the general public. Demonstrates responsible, flexible & dependable behavior in performing duties in a timely manner. Instructs and informs staff, residents and families regarding resident rights and demonstrates support of and compliance with resident rights in all regards including confidentiality of resident information and holds staff accountable to those rights as well. Is responsible to be aware of the Risk Management/Safety Program and the Voluntary Protection Program (VPP), to follow the policy and procedures therein and to promote the effort towards safety facility-wide. Must be able to work in a sedentary position, exerting up to 20 pounds occasionally for lifting, pushing or moving objects. Must be able to sit most of the time (75% of 8 hour shift); standing and walking or able to access the various nursing units intermittently throughout the shift. The ability to provide consultative services to other professional staff who share responsibility for patient care and to social workers in other professional agencies. Will take advantage of additional training relative to the field of social work practice in nursing facilities. Maintains and distributes care conference schedule. Complete Continuous Quality Improvement studies for social services Participate in professional organizations and committees, community programs and networking sessions, as appropriate. WORK AREA A
well-lighted and well-ventilated office with carpeting and a 250-bed skilled nursing care facility with six nursing units. The degree must be obtained within the first 6 months of hire. Maintains a current license by the North Dakota Board of Social Work Examiners or be actively working to obtain licensure. The license must be obtained within the first 6 months of hire. Prior experience in a health-related facility or social service agency dealing with aging A working knowledge of basic social work theory and awareness of state and federal programs and regulations affecting nursing homes and related facilities. In the case of a Social Service Designee, the individual must have a high school diploma or equivalent; must have previous experience as a Social Service Designee in a long-term care setting; must work under the direction of an LBSW. Successfully pass a drug test, background check, including criminal history, certification registries, and employment history Completion of training as required by Missouri Slope.
