Licensed Social Worker SMP Health - St. Catherine North | Fargo, ND SMP Health is seeking a compassionate and resident-focused Licensed Social Worker to join our team at St. Catherine North . In this role, you will serve as an advocate, resource, and support person for residents and their families throughout their long-term care journey. You will work closely with residents, families, and the interdisciplinary team to promote quality of life, support care planning, and ensure person-centered care. Responsibilities Serve as an advocate for residents, supporting resident rights, dignity, and choice Build positive relationships with residents and families Complete psychosocial assessments and participate in care planning Support residents through adjustment, behavioral, emotional, and social challenges Facilitate communication between residents, families, and care teams Coordinate family meetings and care conferences as needed Assist with discharge planning and coordination of community resources Collaborate with nursing, therapy, activities, dietary, and leadership teams Maintain accurate documentation and support regulatory compliance Promote resident engagement, independence, and quality of life programs Qualifications Bachelor's degree in Social Work, Human Services, Psychology, Gerontology, or a related field preferred Licensed Social Worker required Experience in long-term care, healthcare, social services, or case management preferred Knowledge of resident rights, care planning, and long-term care practices Strong communication, conflict resolution, and interpersonal skills Ability to build relationships with residents, families, and team members Strong organizational and documentation skills Ability to work collaboratively within an interdisciplinary team Pay & Benefits $24 - $32.50 / hour or based on experience.
