You will work Monday through Friday, from 8:00 am to 4:30 pm, with a requirement to attend the weekly Interdisciplinary team meeting on Thursday mornings. Your expertise and dedication will be crucial in helping patients navigate their healthcare journeys. Provide comprehensive social work services to patients in a home health setting Collaborate with interdisciplinary teams to develop and implement care plans Assist patients and families in understanding and coping with medical, social, and emotional challenges Ensure compliance with all relevant regulations and standards Maintain accurate and timely documentation using the EPIC charting system Need 1 year experience in setting

Required Certifications:
