Role:
Join STARS Adult Medical Day Care Center as a Social Worker and make a meaningful impact in the lives of our clients. We are looking for compassionate professionals who are dedicated to providing support and guidance in a nurturing environment.
Responsibilities:
Conduct comprehensive assessments to determine client needs and develop individualized care plans. Provide counseling and support to clients and their families to enhance overall well-being. Coordinate with healthcare providers to ensure holistic care for clients. Facilitate group and individual therapy sessions as needed. Maintain accurate and confidential client records in compliance with regulations. Advocate for clients' rights and access to necessary resources and services. Participate in team meetings to discuss client progress and care strategies. Stay updated on community resources and services to better assist clients.
Requirements:
A degree in Social Work from an accredited program. Current state licensure as a Social Worker. Experience in a healthcare or social service setting preferred. Bilingual candidates are strongly encouraged to apply. Strong communication and interpersonal skills. Ability to work collaboratively in a team-oriented environment. Compassionate demeanor with a commitment to client-centered care. Proficient in Microsoft Office and electronic health record systems.
