Role:
As a Social Worker you will play a vital role in supporting the emotional and psychological well-being of our residents. Our community is dedicated to providing exceptional care and services in a warm and welcoming environment. As a Social Worker, you will have the opportunity to work closely with our interdisciplinary team to identify and address the unique needs of each resident, ensuring they thrive and enjoy every day to their fullest potential.
Responsibilities:
Conduct social work assessments to identify and address the physical, emotional, and psychological needs of residents Develop and implement care plans to address the identified needs of residents Collaborate with the interdisciplinary team to provide comprehensive care and services to residents Provide counseling and support to residents and their families to promote emotional well-being and independence Maintain accurate and timely records of resident assessments, care plans, and progress notes Participate in care conferences and other meetings to discuss resident care and services Stay up-to-date with industry developments and best practices in social work and gerontology
Requirements:
Bachelor's degree in Social Work or related field Minimum of years of experience: 2 License or certification in
