Job Overview:
The Social Worker Licensed (LSW) provides direct clinical services in our behavioral health adult outpatient program, such as initial intake assessments, and individual and group counseling services to adults with Serious Mental Illness SMI, including those with a co-occurring substance-use diagnosis in our Adult Outpatient department.
Qualifications:
Required:
Master's Degree in
Social Work Preferred:
LCADC One-year clinical experience in a substance abuse setting
Certifications and Licenses Required:
NJ Licensed Social Worker (LSW)
Scheduling Requirements:
Shift working, Monday-Friday 8am-4:30pm Full-Time (FT)
Essential Functions:
Obtains appropriate release of information Designs and implements treatment plans, including individual, group, and/or family psychotherapy, and evaluates and documents patient outcomes according to department policy and procedure Maintains up-to-date and complete records of patient information according to established standards Assists in reviewing and updating treatment and discharge plans Collaborates with other hospital programs and community agencies with which patients and families are involved Assesses patients' needs through clinical interview, chart/history review and through use of standardized instruments
