Responsibilities Conduct thorough psychosocial assessments of residents' needs upon admission, quarterly, and with any significant change in condition, in accordance with MDS and Care Area Assessment (CAA) requirements. Develop and participate in individualized, person-centered care plans that incorporate crisis intervention strategies, behavioral support, and conflict management techniques. Provide crisis management support during emergencies, behavioral health episodes, or high-risk situations, ensuring safety and stability for residents and staff. Assist residents and families with navigating discharge planning, MLTSS/Medicaid applications, guardianship, and other social service and family law processes. Maintain accurate documentation of resident interactions, progress notes, and case reports in the electronic medical record and using Microsoft Office tools, in compliance with state and federal survey requirements. Collaborate effectively with multidisciplinary care teams (nursing, therapy, dietary, activities, and administration) to coordinate care for residents and families from culturally diverse backgrounds. Facilitate resident and family council participation, grievance resolution, and communication regarding resident rights, ensuring compliance with 42 CFR Part 483 requirements. Support admissions and readmissions coordination, working with hospital discharge planners and case managers to ensure smooth transitions of care. Participate in care conferences, quality assurance/QAPI initiatives, and survey readiness activities as needed. Qualifications Proven experience working with elderly, disabled, or high-risk populations in a skilled nursing, long-term care, or related healthcare setting. Knowledge of resident rights, MLTSS/Medicaid processes, and applicable federal and state long-term care regulations (42 CFR Part 483). Strong communication skills—both written and verbal—to effectively engage residents, families, and interdisciplinary teams, and to document case details clearly. Ability to manage crisis situations with calmness and confidence through crisis intervention techniques. Experience working with individuals with disabilities, dementia, or behavioral health challenges is highly desirable. Bilingual skills are a plus to serve diverse resident and family populations more effectively. Proficiency in Microsoft Office applications and electronic medical record systems for report writing, case documentation, and communication purposes. Bachelor's or Master's degree in Social Work (BSW/MSW) preferred; state social work licensure/certification as required by state regulations for the SNF setting.
