Hospice Liaison Location:
Middlesex County-RWJ University Hospital Schedule:
Full Time Overview:
VNA Health Group is seeking a compassionate Licensed Social Worker (LSW) to join our Hospice Team. The Hospice Liaison plays a key role in assessing patients for hospice eligibility, coordinating smooth transitions of care, and providing emotional and psychosocial support to patients and families.
Responsibilities:
Conduct hospice assessments and admissions in home and facility settings. Educate patients, families, and community partners about hospice philosophy and services. Provide emotional support and counseling during the referral and admission process. Collaborate with physicians, discharge planners, and the interdisciplinary team to ensure continuity of care. Participate in outreach and education to increase awareness of hospice services.
Requirements:
Master's Degree in Social Work (MSW) required Active New Jersey LSW license required Minimum 2 years of experience in healthcare, hospice, or homecare Strong communication, counseling, and organizational skills Valid NJ driver's license, vehicle, and proof of insurance

Services are provided in the patient's home in a community or facility setting In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $60,000 to USD $110,000. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
