Job Description
Social Worker / Case Manager The School Social Worker serves two primary purposes at NECCS. This team member functions as Case Manager for students with Individual Education Plans, ensuring that their needs are met by coordinating the assessment of students by the different specialists, the development of the IEPs, and ensuring that all teachers who are supporting students are aware of their goals, needs, and accommodations / modifications. The second purpose of this position is to provide support to students and families as Social Worker, which may include counseling and connecting families to needed services.
Specific Responsibilities:
Act as case manager for students with Individual Education Plans. Conduct clinical assessments for students being evaluated for special education services Work and collaborate with Behavior Interventionist on Special Education programs Manage and or Provide counseling services as indicated on IEP & as needed in the school. Secure documentation of such services whether managed directly or an external provider Participate in eligibility committee meetings and contributes to the development of the Individualized Education Plan (IEP) with teachers and team Conference with and provides information, support, and counseling to parents/guardians of students. Provides in-service training and workshops for teachers and staff regarding mental health issues, strategies and topics related to meeting the needs of students. Conducts specialized individual and group counseling sessions to address specific emotional, social, and behavioral needs of students. Serves as a liaison between the student, home, school, private counseling facilities and community resources such as social services, court services, and family service agencies. Acts as a member of the school crisis team to provide intervention to students as necessary. Makes court appearances to present data and performs court-requested evaluations. Provide the training and the tools that will enable parents to understand and meet their child's social and emotional needs as it pertains to services provided in their IEP. Maintain an ongoing relationship with families for the purposes of sharing information regarding the student's educational program, assisting the family in cooperatively improving the student's home and school adjustment, using mediation techniques to resolve disputes, providing home management counseling, and assisting the family in utilizing appropriate community and school resources. Consult with and assist instructional staff with program modifications and strategies to encourage and support student personal and social growth and responsible behavior. Serve as the school liaison to all community agencies by consulting and collaborating regularly with school personnel, social agencies, and other community resources to establish and provide coordinated social services to students and families. Participate in faculty, school, parent, and community meetings and programs, sharing observations, issues and resources available. Complete in a timely fashion all records and reports as required by law and regulation or requested by supervisors.
