Job Description

Compensation $56,550.00 per year
Schedule :
M-F 830a-430p Medical or medical stipend. Dental, Vision, Benefit time, 401K Performs psychosocial assessments that are complete and timely: they identify major factors affecting care plan and coping and are completed within 5 days of admission. Develops care plans that are appropriate to meet the needs of individual patients and families and are carried out or revised appropriately. Solves routine care plan problems independently as appropriate. Advocates effectively for services from community organizations. Effectively elicits emotional expression in short-term counseling with patients and families. Meets established goals in a timely manner. Represents the organization's social work department in internal and external meetings, presenting accurate information effectively. Records planned and completed visits, phone calls and meetings in NDoc and other required media in a timely manner. Provides social work counseling to patient and family, with appropriate follow-up visit or phone call to ensure goals are met. Participates in interdisciplinary team meetings and/or individualized conferences. Adheres to employment policies as described in the employee handbook; maintains and respects confidentiality; adheres to all HIPAA regulations. Participates in all required in service education programs and meetings. Demonstrates compliance with the dress code policy by appearing well groomed, wearing ID badge while on duty and maintaining a professional appearance. Performs other duties as assigned.
Requirements:
Required Skills/Abilities:
Knowledge of the dying process; knowledge of policies and procedures of the hospice organization. Knowledge of regulations surrounding compliance with Medicare Conditions of Participation. Ability to function as a resource to patients and families during times of high stress and volatility. Excellent interpersonal and communication skills, including providing support for patients and families, resource providers, members of the clinical team and employees. Ability to comprehend the spoken and written English language, to communicate with clarity both verbally and in writing. Well organized to manage patient and family needs, provide resources and complete timely documentation Ability to work both independently with sound decision-making, and as a part of an interdisciplinary team. Strong assessment and documentation skills. Proficiency in operating a computer; including a working knowledge of Microsoft applications (Word, Excel, and Outlook) and an electronic medical record.
Education and Experience:
Required Education:
Master's Degree in Social Work from a Graduate School of Social Work accredited by the Council on Social Work Education, Current N.J.
License Preferred Education:
Required Experience:
One year of post-Master's Social Work or counseling experience.
Preferred Experience:
Prior experience in a healthcare setting.
