The LSW plays an essential role in supporting patients and their support systems while contributing to integrated, patient-centered care. A typical day for a Licensed Social Worker may include: Assist with Pediatric Transplant evaluations Provide care for Pediatric Nephrology patients in the outpatient setting Assist ESRD/Transplant patients who are part of the practice in the inpatient setting Provide discharge planning, referrals to community agencies and entitlement programs Responsible for overall care coordination of the patient and family from admission through discharge Will be responsible for support groups and office events for CKC patients/families including yearly events Guides the family through a continuum, acting as liaison between the family, a multidisciplinary team, physicians and external agencies Monitors family and patient's understanding and compliance with treatment and discharge goals and coordinates team functions toward achievement of the discharge plan Maintains contact with third-party payers to assure ongoing funding of care and maximum utilization of services available to assure child's ongoing care Responsible for assisting in the social case management of 55 current transplant patients, dialysis patients and 30 recent transplant patients This role might be for you if: Clear, compassionate communication defines how you interact with patients, families, and colleagues You value collaborative teamwork, transparency, mutual respect, and a shared passion for excellence and innovation You are able to plan, organize, and prioritize daily tasks while working with minimal supervision You demonstrate strong clinical judgment and a commitment to evidence-based practice and continuous professional development Providing exceptional customer service and creating a positive patient experience are important to you Adaptability allows you to manage changing priorities while maintaining professionalism To be considered for this Licensed Social Worker opportunity: This position offers a consistent Monday through Friday schedule from 8:00am - 4:30pm, allowing you to build strong patient relationships while maintaining a work-life balance. A Master's Degree in Social Work from an accredited graduate school of social work and a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) depending upon years of social work experience and clinical course work (LSW may render clinical social work services under the direct supervision of an LCSW) is required State licensure is required by the NJ State Board of social work examiners Three or more years' experience in a hospital setting preferred Epic experience and Bilingual Spanish is a plusBenefits
