As a compassionate and dedicated Mental Health Provider, you will provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. Our team is committed to delivering high-quality care and support to seniors, and we are seeking a like-minded professional to join our team. About the Role We are seeking a part-time Social Worker/Care Companion to work approximately 12 hours per week, Monday-Friday, within regular 8-5 hours. As a member of our Collaborative Care team, you will: Visit designated facilities in accordance with the schedule set with your supervisor Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns Document visit notes and maintain detailed records of interactions and interventions after each visit Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP Requirements To be considered for this role, you must have: An Associate Degree in Social Work, Psychology, Counseling, or another mental health-related field Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings Strong communication, documentation, and relationship-building skills At least 1 year of experience in a mental health-related field What We Offer Meaningful, resident-focused work in a supportive team environment Regular supervision and collaboration with psychiatric providers Professional growth opportunities through monthly clinical meetings and ongoing training Opportunity to bring comfort, connection, and care to residents who need it most Flat salary of $20,000 - $22,000 annually Flexible scheduling to fit your needs
