Job Description

Role Overview:
The Medical Social Worker (MSW) has a strong understanding of ALTCS, AHCCCS, and VA financial assistance programs, as well as experience with dementia care. Compassion, advocacy, and a holistic approach to patient and family support are essential qualities for success in this role.
Core Responsibilities:
Conduct psychosocial assessments and develop individualized care plans. Provide counseling and education to patients and families. Coordinate access to financial and community resources, including ALTCS, AHCCS, and VA programs. Advocate for patient rights, preferences, and dignity in care decisions. Support families coping with dementia and complex care needs. Maintain accurate and timely documentation. Participate in quality improvement and interdisciplinary collaboration. Other duties as assigned.
Basic Qualifications/Requirements:
Master's degree in Social Work (MSW) from an accredited program preferred but BSW will be considered Minimum of three (3) years of experience in healthcare or hospice. Strong knowledge of state financial assistance programs, and VA financial assistance programs Experience working with dementia patients and their families Excellent communication, documentation, and teamwork skills. Valid driver's license and reliable transportation. Ability to work effectively as part of an interdisciplinary team. Must be comfortable with death, dying, and providing emotional support to patients and families in crisis. Applicants must be legally authorized to work in the United States.
Work Environment:
Work is performed primarily in patients' homes, facilities, and the community. Regular exposure to patients with serious illness and end-of-life-conditions. May experience emotional stress related to patient care and family interactions. Work schedule may include evenings or weekends to meet patient and family needs. Regular use of personal vehicle required for travel within assigned service area. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of this position.
Travel requirements:
80%- Areas are Maricopa and Casagrande Salary is annualized for reference and paid semi-monthly in accordance with company payroll practices. Employment is at-will.
Physical Requirements:
Frequent sitting, walking, and driving to patient homes, facilities, and community settings. Ability to lift and carry light materials, up to 20 pounds. Ability to communicate effectively in person, by phone, and electronically. Must be able to tolerate variable home environments and emotional situations.
