Schedule:
Monday-Friday, 8:30-4:30 Position Summary UBMD Neurology is seeking a Licensed Master Social Worker to provide clinical & case management services within a well-established neurology clinic. The social worker will be working with adult and pediatric patients diagnosed with Multiple Sclerosis, different forms of Dementia and Huntington disease. Support will also be provided by social workers to the patient's family/caregiver. Essential Functions ➢ Provide full range of patient case management activities. ➢ Address service needs, community resources, patient education to enhance compliance and outcomes and health insurance coverage. ➢ Facilitate referrals. ➢ Advocate for patient needs. ➢ Conduct lethality assessments on as needed basis. ➢ Navigate conflict within family systems that impact the treatment process. ➢ Provide clear documentation of all interactions between pt/caregiver & social worker and keep track to submit for quarterly reports. ➢Conduct psychosocial assessments for pre-symptomatic HD testing. ➢Responsibilities may evolve over time to meet the changing needs of patients and the clinics.
Qualifications Education:
Master's degree in social work, License required. 2 years' practicing as a Licensed Social Worker Past employment working with patients affected/diagnosed with a mental health disorder. Ability to manage crisis situations appropriately. Works well within a multidisciplinary team as well as independently. Good time management and public speaking skills. Ability to succeed in a fast-paced environment. Knowledge of insurance/DSS/community resources/benefits Reliable transportation and willingness to commute to both office locations in Buffalo and Williamsville depending on clinic location and day of the week.
