Job Description
Glens Falls Center seeks a compassionate Social Worker to support residents and families in our skilled nursing and rehab facility. The Social Worker will conduct psychosocial assessments, coordinate care plans, provide counseling, and advocate for resident rights and preferences. This role collaborates closely with nursing, therapy, and medical teams to support safe transitions of care, discharge planning, and community resource referrals. Ideal candidates bring strong communication, documentation, and problem-solving skills and are committed to resident dignity, quality outcomes, and interdisciplinary teamwork in a mission-driven environment. Responsibilities Conduct comprehensive psychosocial assessments for residents and families Develop and update individualized care and discharge plans Provide counseling, crisis intervention, and emotional support Coordinate community resources, benefits, and support services Advocate for resident rights, preferences, and dignity Collaborate with nursing, therapy, and medical teams on care plans Facilitate care conferences and family meetings Maintain accurate, timely documentation and care records Support admission, transition, and discharge processes Participate in quality improvement and compliance activities Required Skills Psychosocial assessment Care and discharge planning Individual and family counseling Crisis intervention Knowledge of community resources Healthcare and LTC regulations knowledge Documentation and EMR proficiency Interdisciplinary team collaboration Resident advocacy Communication and conflict resolution
