Glens Falls Center seeks a compassionate Social Worker to support residents and families in our skilled nursing and rehab facility. The Social Worker will conduct psychosocial assessments, coordinate care plans, provide counseling, and advocate for resident rights and preferences. This role collaborates closely with nursing, therapy, and medical teams to support safe transitions of care, discharge planning, and community resource referrals.
