Job Description

Social Worker/Discharge Planner A well-established Skilled Nursing Facility is seeking a compassionate and driven Social Worker/Discharge Planner to join our team. This role is ideal for someone who is passionate about supporting residents and their families while ensuring a smooth care experience from admission through discharge. Social Worker/Discharge Planner The position requires attending daily morning meetings, weekly UR meetings, leading, planning and completing all discharges. Gathering discharge information from new skilled patients within 48 hours of admission. Planning and attending discharge care plan meetings, addressing discharge concerns, documenting discharge notes in ECS, completing discharge audit forms, assist with annual post form audit. Social Worker is responsible for addressing the psychosocial needs of residents, coordinating care plans, and facilitating safe and effective transitions. This position plays a key role in enhancing resident quality of life and overall satisfaction. Social Worker/Discharge Planner
QUALIFICATIONS/REQUIREMENTS
Bachelor's degree in Social Work or related field (Psychology, Nursing or Care Management); MSW preferred Experience in Social Service office preferred, but not required Prior experience in a skilled nursing facility, rehabilitation center, or long-term care setting preferred Strong communication and interpersonal skills Knowledge of discharge planning and community resources Ability to manage multiple responsibilities in a fast-paced environment Familiarity with EMR systems; PCC and MDS 3.0 experience a plus
Salary Range:
$65,000-$80,000 a year (based on Experience) An Equal Opportunity Employer #boost
