Monday Friday. Flexible schedule. Some evenings hours required.

SUMMARY

Family Service League is seeking a full-time Social Worker to provide clinical services to youth and families through our Children and Family Treatment and Support Services (CFTSS) program. CFTSS is a team-based model which is intended to provide families individualized and flexible services by meeting with children in their homes and other community settings. CFTSS provides treatment to children ages 5-21 and enables a greater focus on prevention and early intervention by providing an array of services and the capacity to intervene earlier in childrens lives.

RESPONSIBILITIESThe Social Worker will provide community based mental health treatment to a caseload of 20 children/youth and their families.

Deliver services with the core values of
CFTSS:
Family-Focused, Community- Based, Multi-Systemic, Culturally Competent, and Least- Restrictive.

The Social Worker will provide evidence-based interventions that are tailored to the childs and familys needs.

Complete treatment plans and implement services based on your assessment and the input of the family.

The Social Worker will complete all documentation in accordance with CFTSS standards in an electronic medical record, including notes, assessments, and treatment plans.

Collaborate with other CFTSS and Care Coordination providers by engaging in regular communication.

Conduct crisis intervention as needed.

Participate in team meetings.

All other duties as assigned. QUALIFICATIONSLMSW, LMHC, LCSW, LMFT, or related licensed clinical degree required. We are willing to consider those with a limited permit or the willingness to obtain the limited permit.

Minimum of 1 year of experience in a behavioral health setting and in a clinical capacity required.

Prior experience diagnosing using the DSM V and experience with behavioral/therapeutic interventions for children, adolescents, and families required.

Proficient computer skills, including Microsoft Office and Electronic Medical Records (EMR) required.

Excellent verbal and written communication skills required.

Valid and clean NYS Drivers License required.

Bilingual in Spanish preferred.

Flexibility to work evenings to meet the needs of children and families required.
PHYSICAL REQUIREMENTS
This position requires sitting for long periods of time, and travelling to different client homes, community settings and FSL offices.
