Full Job Description Social Worker Rate:
$20-$25 (depending on experience)

Days, flexibility required Seneca Health Care Center is seeking a compassionate and dedicated Social Worker to support residents and families through every stage of their care journey. In this role, you'll assess psychosocial needs, coordinate services, facilitate care planning, and help residents achieve the highest possible quality of life in a supportive, resident-centered environment. RESPONSIBILITIES
Complete psychosocial assessments and social histories upon admission Assist residents and families with advance directives, care planning, and transition support Facilitate care plan meetings with residents, families, and the interdisciplinary team Provide counseling, support, and interventions related to emotional, social, and behavioral needs Coordinate discharge planning and referrals to community resources and home care services Complete MDS assessments and documentation as assigned Document resident and family interactions in the electronic medical record Serve as a liaison between residents, families, staff, and outside agencies Participate in QAPI initiatives and contribute to quality improvement efforts Collaborate with all disciplines to promote resident well-being and positive outcomes
REQUIREMENTS
Bachelor's Degree in Social Work, Human Services, Psychology, or related field required LMSW, BSW, or MSW preferred Minimum of one year of experience (paid or volunteer) working with a geriatric population in a healthcare setting Experience with MDS, discharge planning, and electronic medical records preferred Strong communication, organizational, and problem-solving skills Ability to build positive relationships with residents, families, and interdisciplinary team members Commitment to providing compassionate, resident-centered care
