part-time Job Description The Community Partnership on Aging is a unique collaboration between eight cities (Gates Mills, Highland Heights, Lyndhurst, Mayfield Heights, Mayfield Village, Richmond Heights, South Euclid and University Heights) that serves older residents in those cities and throughout Northeast Ohio. The agency supports older adults by providing congregate meals, transportation, social work outreach and referral, homemaking, health/wellness, social and educational programs, food distribution and safe-at-home modifications. Community Partnership on Aging seeks a part time social worker to perform duties including outreach supportive services, information, referral and other social services to older residents of Partnership cities and their families. Position is for 30 hours a week; hours are flexible. Paid fringe benefits include Health Insurance, OPERS, Worker's Comp and Medicare. www.communitypartnershiponaging.org Salary range $24-$28 hour

Essential Functions:
  • Conduct client-find/assessment interviews, developing an appropriate client care plan, if indicated and as appropriate.
  • Ensure client care plans are re-assessed at specified intervals, per program requirements, and as needed, for clients who are actively participating in agency services.
  • Provide supportive services, outreach advocacy, information and referral, and other social services as appropriate. Make referrals to other agencies, as needed.
  • Provide individual and family supportive counseling as indicated by care plan. Make referrals for therapeutic counseling to local providers as appropriate.
  • Complete case notes, accurately recording client contacts, and recording coordination with family and with other agencies on clients' behalf.
  • Maintain positive working relationships with community service providers, staff, volunteers and clients
  • Maintain up to date knowledge of available services within each of the member cities.
  • Participate in special programming and activities offered by CPA during off hours, with advance notice.
  • Assist in field education for any student training at CPA.
  • Attend continuing education workshops, as related to clients and responsibilities. Cognitive, educational and language requirements

  • Ability to use agency software system in recording, maintaining and reporting data
  • Ability to solve problems independently and handle emergency situations
  • Displays sensitivity to aging population
  • Demonstrates ability to establish and maintain positive relationships with clients, colleagues and community partners
  • Proficient in the use of Microsoft 365 (Word, Excel, Outlook) The Community Partnership on Aging requires applicants to complete a pre-employment screening process upon receipt of an offer of employment.
