The Social Worker 3 role within University Hospitals Behavioral Health offers a unique chance to become part of a dynamic, collaborative interdisciplinary team. Joining our team comes with several benefits, including: Free parking for regular status caregivers Tuition reimbursement Full benefits and retirement Support for continuing education, career growth, and professional development A Brief Overview Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans. Serves as a resource within the social work department and the hospital. What You Will Do Provides psychosocial assessment / intervention to assist patients/families (including clinically complex).
  • Provides age specific care to patients/families in all age groups. Develops, documents and implements a comprehensive plan based on identified psychosocial needs and/or discharge needs.
  • Develops plans with attention to individual patient and family goals
  • Provides linkage to community resources and UH network services to facilitate optimal care; and serves as liaison to community agencies.
Communicates/collaborates with the work team using a multidisciplinary approach. Provides guidance and serves as consultant to staff and other disciplines regarding complex psychosocial and/or discharge planning issues/barriers. Assists with recruitment, and orientation/mentoring/education of new staff and/or social work interns. Additional Responsibilities Performs other duties as assigned. Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace. Qualifications Education Master's Degree from an accredited school of Social Work (Required) Work Experience Hospital/Health Agency experience. (Preferred) Knowledge, Skills, & Abilities Computer skills (Required proficiency) Theoretical and clinical knowledge regarding health care delivery (Required proficiency) Multi-tasks and prioritizes work (Required proficiency) Works autonomously (Required proficiency) Communicates effectively with persons of various backgrounds (Required proficiency) Licenses and Certifications Licensed Independent Social Worker (LISW) (Required) Licensed Independent Social Worker Supervisor (LISW-S) (Preferred) Position Details/Preferred Qualifications This outpatient Social Worker will be seeing adult patients with various mood disorders, including depression, anxiety, bipolar, etc. Previous experience working with this patient population is strongly preferred. Must be proficient in performing diagnostic assessments, treatment planning, documentation, etc. Having an LISW-S is preferred. Knowledge of Epic is a plus. Hours are 9:00am-5:00pm but has the option to work until 6:00/6:30p if there is a desire to lead a group session (i.e. long-haul Covid, disordered eating, etc.).
