Licensed Social Worker (LSW)Join a Team Where Your Expertise Makes a Difference Are you a Licensed Social Worker who wants more than a traditional case management role? We're seeking a compassionate, detail-oriented clinician who is passionate about improving resident outcomes, collaborating with interdisciplinary teams, and helping shape best practices within a skilled nursing environment. In this role, you'll have the opportunity to use your clinical expertise to support residents, families, and care teams while contributing to the overall quality of care delivered throughout the facility. What You'll Do Complete and oversee MDS and PHQ-9 assessments, ensuring accuracy and regulatory compliance. Coordinate and manage discharge planning, helping residents transition safely and successfully to their next level of care. Conduct social service and recreational assessments to support resident well-being and quality of life. Collaborate with physicians, nursing staff, therapists, residents, and families to develop comprehensive care plans. Provide recommendations that support positive resident outcomes and enhance care delivery. Assist the facility in identifying and implementing best practices related to resident care and social services. Maintain accurate documentation in accordance with federal, state, and facility guidelines. Support residents and families through advocacy, education, and resource coordination. Perform additional duties as assigned. Qualifications Required Current Licensed Social Worker (LSW) license in the State of Ohio. Knowledge of social work principles, practices, and applicable regulations. Experience with MDS assessments and PHQ-9 documentation. Strong communication, organization, and interpersonal skills. Preferred Experience in a skilled nursing, long-term care, rehabilitation, or post-acute care setting. Knowledge of discharge planning and care coordination processes. Why Join Us?
