Renal Social Worker

Summary/Objective Concerto is seeking a Social Worker to join its team! The Social Worker provides psychosocial evaluation and counseling to patients who reside in a Skilled Nursing Facility or rehabilitation center regarding dialysis related issues; works in collaboration with the multidisciplinary treatment team to incorporate psychosocial needs/concerns in planning care; documents on patients in dialysis chart in keeping with facility policies to meet Medicare and Quality Assessment Performance Improvement (QAPI) requirements. This position requires facilitation, as needed, between the dialysis unit and the Social Services Department of the SNF. The Social Worker functions as a member of the Interdisciplinary team. Reports directly to the Manager of Social Services. Essential Functions Conduct psycho-social assessments on each patient and determine social work treatment plan compatible with a total medical treatment plan per CMS guidelines (initial, 90-day, annual and as needed for unstable patients). for dialysis patients and coordinate POC meetings with RN, RD, MD and patient. Complete KDQOL, PHQ9/2, and other tools as required Complete Quarterly Reviews and provide quarterly documentation in every patients charts Provide an education packet to new patients regarding their Rights and Responsibilities, Grievance Procedure, treatment options, and the importance of treatment, and complete documentation that this was completed. Complete monthly tracking of patients needed for QAPI. Maintain professional working relationships with other staff of Concerto as well as in the skilled nursing facility. Participate in administrative and staff programs as required, including attending staff meetings and in-service meetings. Compile and develop educational materials to be used as teaching aids in education of patients and staff as needed. Special projects as needed Masters degree in social work. Healthcare or dialysis related background preferred Must be licensed as an LSW or LCSW. Unencumbered drivers license. Intermediate computer skills and proficiency with MS Word, Excel, Outlook as well as becoming proficient with Concerto specific applications within 60 days of employment. Physical Demands of the Job Must be able lift at least 20 lbs. Requires mental, visual, and auditory attention to patients, customers and staff. Work environment is characteristic of a health care facility with air temperature control and moderate noise levels. May be exposed to infectious and contagious disease/materials. Requires travel between facilities.
