6-11-26 Develops, plans, and implements strategies for program evaluation and growth. Organizes and coordinates post-intervention follow-up with individuals who engage with the Mobile Crisis Response Team. Supervises crisis response teams.ESSENTIAL DUTIES AND RESPONSIBILIITES
This job description is not intended to be all-inclusive. Employee will also perform other reasonably related duties assigned by immediate supervisor.- Response in person to a mental health crisis with a case manager/peer supporter.
- Organizes and coordinates post-intervention follow-up with individuals experiencing a mental health crisis.
- Provides community resources and makes referrals, schedule follow-up services, navigate financial/insurance issues, and remove barriers to receiving services.
- Provides in-the-moment supervision and consultation to staff working with individuals in crisis.
- Collaborates with law enforcement and various mental health providers while maintaining positive relationships with community professionals.
- Maintains a working knowledge of the mental health and human service systems in our region.
- Completes all required charting, casework, documentation, and computer entry, on a timely basis.
- Creates patient treatment plans.
- Provides oversight of the Mobile Crisis Response Team.
- Engages in staff development and evaluation.
- Ensures the documentation and quality of client specific and program data.
- Oversees orientation and training of staff members.
- Answers calls for service from the internal crisis line.
