A Family, School, or General Social Worker provides specialized assistance to vulnerable families and children. Intervenes in crisis situations; evaluates individual and family needs, helps clients get access to health care, mental health care or social services such as housing assistance. Advocates for clients and provides follow up visits. May work with teachers, parents and school administrators to help students improve academically and deal with social issues such as bullying.

Job Description

ESSENTIAL DUTIES AND RESPONSIBILITIES
Help plan and implement social services programs and activities. Assist with coordinating social services activities with administration, medical and nursing services, Consultants, and other related departments. Ensure that necessary forms, reports, and progress notes are properly completed and contain informative descriptions of the services provided and the patients' responses to the services. Participate in community planning, development, and implementation of social care plans, patient assessments, and discharge planning to meet the needs of the hospital, the patients, and their families. Coordinate social services programs with the patients, their families, staff members, and community agencies through interviews, consultations, and direct involvement in the programs. Ensure compliance with employment laws and company policies. Assist in developing, for each patient, a preliminary and comprehensive assessment and written care plan that identifies the emotional and social problems and/or needs of the resident and the goals accomplished for each problem and/or need identified. Encourage patients and families, where appropriate, to participate in the development and review of care plans. Maintain patient confidentially. Know and comply with patients' rights regulations. Ensure the patients' emotional and social needs are met. Review and promptly report to the Director of Social Services and maintain a written record of all patient complaints and grievances. Coordinate social services with all other patient care services to assure a smooth integration to quality resident care. Maintain a working knowledge of applicable government regulations and ensure compliance.
QUALIFICATIONS
Must be a licensed social worker in the State of Ohio. Must have equivalent social work experience. Must have a minimum of one (1) year of related experience in a healthcare setting. Must be capable of maintaining regular attendance as required. Related social work experience at a level necessary to accomplish this position.
PHYSICAL DEMANDS AND ENVIRONMENT
Frequent walking, reaching, climbing, bending, lifting, grasping, fine hand coordination, pushing, and pulling. Willing to work beyond normal working hours and during disastrous circumstances, if needed. Must be capable of performing the Essential Job Functions of this position with or without reasonable accommodation.
