We are seeking a caring and organized Social Worker Designee to support residents and families with psychosocial needs, admissions, discharges, and care planning. The Social Worker Designee assists in managing the day to day social services operations. This role supports residents and families through transitions, care planning, problem-solving, and emotional support. The ideal candidate is compassionate, detail oriented, and comfortable working with seniors and their families.
