Job Overview We are seeking a compassionate and skilled Hospice Social Worker
  • LMSW or LCSW to join our interdisciplinary care team. You will provide psychosocial support, resource coordination, and guidance to hospice patients and their families based on individualized assessments. This role plays a vital part in ensuring quality end‑of‑life care and supporting patient dignity and comfort.

  • LMSW or LCSW Key Responsibilities Conduct psychosocial assessments for hospice patients and families Identify emotional, financial, environmental, and community resource needs Participate in Interdisciplinary Group (IDG) meetings and care planning Provide ongoing counseling, education, and support to patients and caregivers Make appropriate community referrals to meet patient needs Maintain accurate documentation and meet agency/board continuing education requirements Hospice Social Worker•LMSW or LCSW Minimum Requirements Graduate of an accredited Master's degree in Social Work program required Current
LCSW/LMSW
License required Valid driver's license, reliable transportation, and current auto insurance Minimum 1 year of social work experience in a healthcare setting Strong communication and interpersonal abilities Knowledge of community resources Work Environment Care provided in patient homes under varying conditions Exposure to infectious diseases, blood, and bodily fluids possible Requires local travel and ability to work a flexible schedule May involve prolonged standing, walking, and lifting up to 50 lbs
