Full-time Job Summary We are seeking a compassionate and experienced Hospice Medical Social Worker to join our interdisciplinary hospice team. This role provides psychosocial assessments, counseling, and care coordination to hospice patients and their families, supporting emotional, social, financial, and environmental needs during end-of-life care. Responsibilities Complete psychosocial assessments within 5 days of hospice admission Identify psychosocial needs and develop interventions for the plan of care Participate in Interdisciplinary Group (IDG) meetings Provide counseling, crisis intervention, and emotional support Document assessments and interventions in compliance with HIPAA and CMS regulations Collaborate with nurses, physicians, chaplains, and other team members Qualifications Bachelor's or Master's degree in Social Work Minimum 1 year hospice social work experience (required) LSW or LCSW preferred Experience with counseling, patient assessments, and documentation Strong communication and interpersonal skills Benefits Health, dental, vision, and life insurance IRA matching Paid time off Mileage reimbursement Flexible schedule Apply today to make a meaningful difference in the lives of patients and families.
