Part-Time Social Worker - Bethlehem, PA Are you a compassionate and dedicated Social Worker looking for a flexible opportunity to support individuals and families in your community? Soliant is currently seeking a Part-Time Social Worker to provide case management and care coordination services in the Bethlehem, PA area. This position is ideal for a professional who values flexibility while making a meaningful impact through client advocacy and support.

Position Details:

Part-Time Opportunity (Approximately 2 days per week) Flexible Schedule - Choose the days that work best for you

Location:

Bethlehem, PA Area Primary focus: Case management, resource coordination, and client support Pennsylvania Social Work licensure (LSW/LCSW) preferred; qualified candidates with relevant experience are encouraged to apply

Key Responsibilities:

Provide case management services to support client needs and goals Complete assessments and assist in developing individualized support plans Coordinate referrals and connect clients with community resources and services Collaborate with families, healthcare providers, and multidisciplinary teams Maintain accurate documentation and case records Monitor client progress and advocate for appropriate services and supports Promote effective service delivery while following professional and ethical standards
