Job Description

We are seeking a compassionate and dedicated Social Worker to join our Skilled Nursing Facility team. The ideal candidate will support residents and their families by providing emotional support, discharge planning, care coordination, and advocacy to ensure the highest quality of care and quality of life.
Responsibilities:
Conduct resident assessments and develop individualized care plans Provide counseling and emotional support to residents and families Coordinate discharge planning and post-acute services Collaborate with nursing, therapy, and interdisciplinary teams Ensure compliance with state, federal, and facility regulations Assist with admissions, transitions, and resident advocacy
Qualifications:
Bachelor's or Master's degree in Social Work (BSW/MSW) Current state licensure (as required) Experience in a skilled nursing or healthcare setting preferred Strong communication, organizational, and interpersonal skills Compassionate and resident-centered approach
