Qualifications Education:
Master's Degree from an accredited college or university, in Social Work or a clinical related mental health field.
Experience:
Requires a minimum of two (2) years in a psychiatric health care facility or equivalent, with direct experience in individual, family and group therapies, crisis intervention, and treatment skills; must have strong clinical assessment skills.
Position Responsibilities:
Assesses and identifies common recurrent patient problems, symptoms, and behavioral changes. Assesses, obtains and reviews available data obtained by other members of the treatment team. Answers assessment calls promptly, establishes initial rapport with caller, gathers and documents information, assesses needs of call and schedules intake assessment. Assesses problem areas specific to the patient population & provides input into areas needing improvement. Demonstrates skill in establishing rapport with patients who are depressed, withdrawn or otherwise resistant to treatment. Documents findings on the admission/psychosocial assessment. Documents assessment update when the patient is transferred into another level of care and on readmission's in the last thirty days. Consult with families on emotional, social and financial aspects of care. Identifies potential problems/safety risks and demonstrates strong behavior management skills. Assesses and identifies the needs of the patient and family for teaching as it relates to the patient/family system. Assists in the development of an interdisciplinary treatment plan using assessment data from physician, nursing, social work and other activities. Documentation in the progress notes and on the treatment plan up date clearly reflects patient/family progress made towards goals and response to interventions. Participates in departmental staff meetings offering ideas and suggestions regarding goals, objectives and plans for attaining these goals in regard to program issues. Manages time appropriately; able to establish priorities; demonstrates initiative in carrying out responsibilities follows assigned task through to completion in a timely manner. Performs other duties as assigned/required by this position.
