What You'll Do:
As our Residential Living Social Worker , you'll play a vital role in helping residents navigate life's transitions while promoting independence, wellness, and quality of life.
Your responsibilities will include:
Coordinating admissions and welcoming new Residential Living residents Serving as an advocate and trusted resource for residents and their families Assisting residents with transitions, discharges, and moves throughout the community Collaborating with our Wellness Team, Personal Care, Skilled Nursing, Marketing, and Life Enrichment departments Helping resolve resident concerns while promoting independence and resident satisfaction Planning wellness programs, educational opportunities, support groups, and resident events Communicating with outside providers and coordinating services to meet resident needs Monitoring resident well-being, documenting incidents, and ensuring compliance with community policies ✨ Supporting a warm, engaging, and person-centered environment where residents feel valued every day Why Peter Becker Community?
