DESCRIPTION OF WORK
This position oversees key care management activities that support older adults throughout the community. It ensures that services are well-coordinated and delivered in accordance with program requirements. As an Aging Care Management Supervisor 1, you will perform the following duties:
Staff Supervision:
Supervise Aging Care Managers who provide care management to older adults
Eligibility Review:
Determine client eligibility for programs and make appropriate referrals
Report Preparation:
Prepare monthly, quarterly, and annual reports on department status
Service Monitoring:
Monitor services under contract such as home support, personal care, and legal services
Agency Coordination:
Maintain contact with community agencies and exchange information on services for the elderly
Work Schedule and Additional Information:
Full-time employment
Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with 60-minute lunch.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY QUALIFICATIONS
Minimum Experience and Training Requirements:
Three years of experience in public or private social work and a Bachelor's Degree; or
Any equivalent combination of experience and training including successful completion of 12 semester hours of college level courses in sociology, social welfare, psychology, gerontology, or other related social sciences.
