Summary According to social work practice and professional standards, works as a member of the healthcare team to facilitate continuity of care by assessing, planning, coordinating and implementing discharge planning appropriate to the patient's level of care and post-discharge needs. Provides supportive counseling, crisis intervention, financial assistance, and information and referral services to both internal and external customers. Minimum Qualifications Master's of Social Work degree (MSW). Social Work license in the state of Pennsylvania or license eligible; must receive license within six months of hire. Strong verbal and written communication skills. Strong negotiation skills and the ability to address complex issues with patient/family and staff across multiple departments. Ability to work independently and to gather pertinent patient information. Preferred Qualifications Minimum of 1-2 years experience in an acute care hospital setting. Fundamental understanding of personal computers and familiarity with the Windows operating system.
