Job Description

Hours:
Monday - Friday 8:00 - 4:30, Weekend Call and 1 Holiday per year Join our team as a Social Worker! We are seeking a dedicated Social Worker to provide comprehensive psychosocial assessments, crisis intervention, and counseling for individuals, families, and groups. Your role will include bereavement and end-of-life support, as well as timely discharge planning to meet anticipated needs. Primary Responsibilities:
  • Provide resource-specific information and facilitate referrals for patients and families.
  • Mentor social work students and serve as a field instructor for MSW students.
  • Utilize therapeutic counseling methods, including crisis intervention and psychosocial support.
  • Educate patients and families on reimbursement systems and advocate for continued care authorizations.
  • Assess and intervene with medically and psychosocially complex patients.
  • Monitor and reassess the plan of care throughout the hospital stay.
  • Collaborate with the healthcare team and community resources to develop individualized care plans.
  • Educate and advocate for patients and families on medical care decisions, discharge planning, and end-of-life care.
  • Complete comprehensive psychosocial assessments considering various factors and support systems.
